Client Portal & Forms


After contacting me and making your first appointment, you will be sent a link via email to your secure, confidential Client Portal where you will complete all initial intake and payment forms electronically. Completing your intake documents online may take approximately 15-30 minutes. All forms must be completed within 24 hours of receiving the Client Portal email in order to hold your appointment time. If documents are not completed within 24 hours of receiving the email, your appointment time will be released. Your secure Client Portal allows you to pay balances online, update credit card information, obtain/view invoices, keep track of appointment history, as well as review all documents you have signed and completed. You are granted access to your Client Portal for as long as you are a current client.

This page includes links to a few of the documents you will be asked to review prior to being seen. The information in each of these forms will be referred to and discussed during your first appointment. Therefore, it is important that you already have an understanding of their content prior to coming in for the first time and are prepared to ask any questions you may have.
